We are very excited that you are planning to visit us here at Smileworks Dentistry!

As with most health-related processes, dentistry requires a little bit of paperwork to be filled out, but we've cut it down to only about 2 & 1/2 pages, so don't worry!

Better yet, you can download a copy right on your computer, print it, fill it out as best you can before coming in, and come visit us, ready to receive instant dental care!
